AccessLife Support Services (“we”, “our”, or “us”) is committed to protecting your privacy and handling your personal information responsibly, in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and the NDIS Practice Standards.

We may collect personal and sensitive information including:

• Name, date of birth, contact details
• NDIS number and plan information
• Medical, psychological or behavioural reports
• Information about your goals and support needs
• Details provided by support coordinators, family members, or other providers
   

2. How we collect your information

We may collect your information through:

• Referral forms
• Phone or email communication
• In-person meetings
• Documents provided by you or a trusted representative
   

We only collect what is necessary to provide safe, high-quality services.

3. Why we collect your information

We collect your information so we can:

• Assess your needs and eligibility for services
• Develop support plans tailored to you
• Coordinate supports and communicate with relevant stakeholders (with your consent)
• Meet legal, contractual, and regulatory obligations
   

4. How we store and protect your information

We store all information securely through Google Workspace for Business, which uses industry-standard encryption. Paper documents (if used) are stored securely and accessed only by authorised team members.

We take all reasonable steps to protect your information from loss, misuse, or unauthorised access.

5. Who we share your information with

We only share your information:

• With your consent, or the consent of your guardian
• With staff or contractors involved in your support
• With plan managers, other providers, or relevant services as required
• If legally required (e.g. in cases of risk, abuse, or subpoena)
   

We will never sell your information or share it for marketing.

6. Accessing or correcting your information

You can request to access or update your personal information at any time by contacting us. We will respond within a reasonable timeframe and support you through the process.

7. Making a complaint

If you are concerned about your privacy, you can contact us directly. We take all complaints seriously and will work with you to resolve any issues.
